Complete Evaluation in Four Steps

01.

Share Your Program Details

Tell WA PREP what curriculum you are using, when instruction will happen, and how many young people and educators you expect to participate.

02.

Confirm Your Evaluation Process

Review the consent, survey, and submission instructions provided by WA PREP before programming begins.

03.

Use the Correct Surveys

Survey versions vary by grade, age, and program setting. Use the unique online survey links assigned to your organization.

04.

Submit Required Materials

Complete the evaluation checklist and submit required surveys, attendance information, and other materials as instructed by WA PREP.

Before Teaching Begins

Before you begin instruction, please provide WA PREP with:

  • Curriculum being implemented
  • Estimated instructional hours
  • Estimated number of young people participating
  • Estimated number of teaching staff
  • Planned implementation date or dates

Providing this information early helps the evaluation team prepare the correct materials for your program.

Consent and Voluntary Participation

Before programming begins, follow the parent or guardian notification and consent instructions provided by WA PREP for your setting.

Taking a survey is always voluntary. Young people may skip any question or choose not to complete a survey. They may still participate fully in PREP programming.

Contact WA PREP before implementation if you are unsure which consent or notification process applies.

Finding the Right Survey

Survey Collection Note: Downloadable surveys are provided for preview. Participating partners should use the unique online survey links and collection instructions provided by WA PREP.
