Evaluation Guidance

Thank you for participating in WA PREP!

Any of the Evaluation requirements you need can be found below.

We are excited to work with you in providing comprehensive sexual health education (CSHE) to young people across Washington. Below we outline the process that facilitators will follow based on age and location type. The process is different for ages and sites, because federal requirements for data collection vary based on program setting. Please read these instructions carefully and follow them as closely as possible.

Prior to teaching, WA PREP will need the following information from each partner:

Curriculum being implemented
Estimated hours of teaching time
Estimated number of young people taught
Estimated number of teaching staff
Date that Implementation will occur

Obtaining Consent for Entry and Exit Surveys

Youth aged 17 or younger must have passive consent of a parent or legal guardian to participate in PREP programming. Passive consent means notifying the parent/guardian of the PREP program prior to the start of the program and asking them to only respond if they do not want their child to participate. Even if parents do provide consent, youth always have the right to not take the surveys. Youth who choose not to do the surveys can still participate fully in PREP programming. All survey data collection is voluntary.

Partners will be given links to online surveys they can access for their district, but below are PDFs of each survey for preview purposes.

Confidentiality Procedures

WA PREP respects and wants to ensure that the privacy of your youth is maintained. All evaluation instruments (i.e. surveys) are confidential. The survey links are all anonymous. If you decide to collect paper surveys, please communicate to youth that these are anonymous and they should not put their names, initials or any other personal information anywhere on the surveys. Please review the surveys for any identifying information and black out the information before submitting to WA PREP.
